Guidelines parents and students should know when interested in youth employment

Student Working Paper Requirements:

  • SIGNED APPLICATION
    (APPLICATION MUST BE SIGNED BY PARENT OR GUARDIAN)

  • EVIDENCE OF AGE
    (BIRTH CERT., BAPTISMAL CERT., PASSPORT, RESIDENT ALIEN CARD OR STATE ID)

  • PROOF OF A PHYSICAL
    (PHYSICAL MUST BE DATED WITHIN ONE YEAR OF TODAY’S DATE)

  • STUDENT MUST APPEAR IN PERSON TO PRESENT DOCUMENTS
  • Application MUST BE SIGNED BY YOUR PARENT where indicated
  • Obtain Application  (Form AT-17) from: 
    Census Office 3rd floor City Hall
    OR Health Office at your school
  • Complete PART I ONLY of the Application

    ITEMS YOU MUST BRING IN TO OBTAIN YOUR WORK PERMIT:
  • SIGNED APPLICATION (Form AT-17) PART 1 ONLY 
  • PROOF OF PHYSICAL EXAMINATION within the last 12 months:
    • Form from your PRIVATE PHYSICIAN
      OR
    • Form from your SCHOOL NURSE
      (Immunization records WILL NOT be ACCEPTED)
  • PROOF OF YOUR AGE – BRING ANY ONE OF THE FOLLOWING DOCUMENTS AS PROOF:
    • Birth or Baptismal Certificate
    • Passport
    • Learner’s Permit or Driver’s License
    • Resident Alien Card

WORK PERMIT APPLICATIONS ARE PROCESSED:

September 1 to June 30 - Monday – Friday 2:00 - 4:00 p.m

During School Vacations & July 1 to August 31

Monday – Friday 10:00 a.m - NOON and 2:00 p.m - 3:00 p.m
